REFERRAL PLAN

With access to a network of thousands of medical facilities and physicians nationwide, CAREONE BEST BENEFITS members can save on medical services. Members who have no health care benefits can save money on all visits to a participating provider. Members who do have health care benefits can save on services not covered, such as routine office visits and cosmetic surgery. At discounts of 5% to 40%, members can save hundreds or even thousands of dollars a year.

Participating providers are screened by a team of professionals and must pass strict criteria prior to joining the network. All providers are licensed and credentialed. There is no paperwork to complete and there is no limit to the number of visits to your provider. Simply make an appointment with a participating provider from the list in your membership kit or contact a Customer Service Representative at 1-800-308-0374 for the name of a provider in your area. You may also use the Internet to search for participating providers online at: http://beechplus.beechstreet.com/MWAME/ (Case Sensitive)

Present your identification card and the “Dear Doctor” letter included in your membership kit to the receptionist when you arrive at the participating physician’s office.

For services performed by the physician, you will pay the reduced fee at the time of your visit .For services performed in the hospital, (only if “Hospital” is on your identification card), you will receive a billing statement detailing your savings and the amount you must pay within 30 days of services.

Remember, this is not an insurance plan and there will be no reimbursement to you or to the participating provider. You are fully responsible for paying the provider directly for services rendered.

This plan is provided through one of the largest provider networks in the nation. The network is available throughout the United States and providers are continually being added to meet your member needs.

ATTENTION MARYLAND RESIDENTS The Hospital Referral Plan benefit is not available in Maryland. Discounts on Hospital services are not  allowed under Maryland law.  Discounts on Physician services are eligible for the discount on the program.
