Q: How does the CAREONE Best BENEFITS plan work? A: Each CAREONE Best BENEFITS member receives discount prices when he/she uses a participating provider.
Q: Who can qualify as a dependent under my plan? A: The plans include the immediate family. Q: Is there a limit to how many dependents I can have on my plan? A: No, there is no limit for dependents.
Q: How can I add or remove a household member from my plan? A: Simple, Just submit a written notification stating the household member you wish to add or remove.
Q: How can I cancel my membership? A: You may cancel your membership at any time be submitting written notice of cancellation either by mail or by facsimile to CareOne Best Benefits. Please include member's name and member ID number. You may also call the Best Best Benefits plan at 1-800-308-0374. CAREONE Best BENEFITS will cancel your membership immediately upon receiving your request. Q: Can I get a refund if I decide to cancel my plan? A: In the event that you wish to cancel your membership during the first thirty days of your enrollment date, you will receive a full refund of your membership dues.
Q: What is the CAREONE Best BENEFITS privacy policy? A: CAREONE Best BENEFITS respects your privacy. We only use your information to identify you, and will not share or sell that information ever. Except for details needed to fill your prescription, CAREONE Best BENEFITS will not knowingly provide your personal information to anyone. Vendors who provide services such as filling orders, delivering and processing payments are obligated to use your info strictly for those purposes. CAREONE Best BENEFITS is not responsible for privacy policies of other providers or advertisers, such as pharmacies. We encourage you to review their privacy policies. We use strong security to protect against unauthorized access. . . Q: Can I choose my doctor? A: Yes, but you must choose a doctor from our network. Our network is one of the largest in the nation with 400,000 providers to choose from. Go online to www.careonebenefits.com (provider locator) or call 1-800-308-0374 to find healthcare providers. Q: What if my doctor is not on the list? A: That's easy! All you have to do is submit in writing the name, address, and phone number of your doctor or dentist to our provider relations department via fax, e-mail, or regular mail. We will invite him or her to become a CAREONE Best BENEFITS provider! Note: This process is a courtesy to our members, not a guarantee that your provider will join CAREONE Best BENEFITS or agree to give a discount. Q: Can I use this service if I have medical insurance and/ or Medicare? A: You can always use your CAREONE Best BENEFITS plan to save on the many prescriptions drugs not covered by other plans. The CAREONE Best BENEFITS program will provide benefits for medical costs that are excluded or restricted conditions under your benefits plan. Additionally, if you have a Health Savings Account or Medical Savings Account the CAREONE Best BENEFITS program can significantly reduce your out-of-pocket expense for Health care costs. The CAREONE Best BENEFITS program is also effective for uninsured or underinsured patients. It is not intended to cut already discounted prices provided by prescription drug plans. Even so, it is worth checking to see whether shopping with CAREONE Best BENEFITS plan can save more than paying the insurance price. Q: How much do I have to pay when I go to the doctor? A: The savings vary from one provider to another so frequently, that there are no averages or minimums. The savings is determined by the agreement we have with our provider. The savings will be applied, then the provider will expect the payment for charges at the time of service.
Q: How do I know how much I will be paying for dental procedures? A: Our providers follow a set fee schedule that designate the discounts for most procedures. You may contact CAREONE Best BENEFITS Member Service for an estimated cost for a particular procedure or contact your Dentist directly.
Q: If I cannot afford to pay my entire bill up front, will the doctor make payment arrangements? A: Many providers will arrange a payment plan. However, if you do not pay your bill at time of service the provider is not obligated to honor the lower rates.
Q: How do I use the Prescription Plan? A: After you have received your CAREONE BENEFITS membership card, all you do is present it to one of the participating pharmacies. You will receive the lower rate for your prescription(s) at the time. There are no forms to fill out for reimbursement. Saving is applied at the time of service.
Q: Do I need a prescription? A: Yes. A valid prescription from your doctor is required to place an order using the CAREONE Best BENEFITS on line system.
